Advance Care Planning Now Offered at the Cancer Community Center

Tara Baysol, Certified Advance Care Planning Facilitator, is available to meet with people whose lives have been disrupted by cancer and would like to talk about this important topic. She will be providing this service at the Center through the end of April.

Advance care planning is the process of thinking and talking about future healthcare decisions if you had an event, like an accident or illness, and could not make your own decisions.

A private, one-hour advance care planning session at the Center will help you:

  1. Explore beliefs that inform your goals, values, and preferences
  2. Choose a healthcare agent to help make healthcare decisions if you are unable to make them yourself
  3. Decide on the type of care you would like to receive if you have health complication

Cancer can make you feel like you have lost control.

You can take control of your future health care needs with advance care planning.

Schedule your appointment with Tara Baysol, Certified Advance Care Planning Facilitator by calling the Center at 207-774-2200.
